We are excited to announce that we are now registering for BASCOL's Summer Program 2025! Dear Prospective BASCOL Families, The summer program will be located at Millard Hawk Elementary School (74 School Drive) for Weeks 1-9. *Space is limited and will fill up very quickly. The summer program will run from June 30th through August 27th, 2025 from 6:30am until 6:00pm Monday-Friday. You choose only the days and weeks you need to attend (minimum of 2 days per week). There are themed weeks, guest speakers and field trips! Please note: Field Trips are not optional. If you sign up for a field trip day, you must also sign up for the field trip. There will not be any staff or children staying back from the field trips. Ways to register:
- Complete the attached Summer Registration Packet and e-mail back to mmanning@bascol.org.
- You may also fax your completed packet to 315-622-4885.
- Or you can mail completed packet to BASCOL, 4610 Wetzel Road, Liverpool, NY 13090.
- You can also call 315-622-4815 and make an appointment to register in person at the BASCOL Administrative Office located at 4610 Wetzel Road in Liverpool.
(If you fax or mail in your packet, please call the BASCOL office to confirm receipt between 7:30am-5:30pm Monday-Friday.) We will let you know when we have received your packet and follow up with you about any missing information needed. The registration fee is $40 per child if you register before May 30th ($50 per child after May 30th). You will also pay for the field trip t-shirt upon registration if you are purchasing one. You can use last year's t-shirt. Payments for the summer will be done through our billing system, Procare. Autopay will be required- see memo below regarding Procare. We can also mail you a paper packet if you prefer. Please don't hesitate to ask. We must receive a completed Registration Packet along with any other required health care forms, registration fee and deposit before your registration is all set. IMPORTANT PLEASE READ BELOW: If your child has an Individual Education Plan (IEP) or 504 Plan, that will also need to be submitted when you register. In addition, if your child has ADHD – please let us know if they will require any special accommodations @ BASCOL. If any allergies to food, latex, insects, allergy to medications and seasonal allergies are listed, the OCFS 6029 Allergy & Anaphylaxis form will need to be filled out by your child’s doctor. If medication is required at the program a Medication Consent Form would need to be completed as well. See forms below. For any sensitivities or intolerances (to foods, including dairy, latex or insects that are listed, that do not require a medication and have possible symptoms of an allergy reaction with the potential of anaphylaxis, According to NYS Office of Children and Family Services (NYS OCFS) Regulations Part 414, and pursuant to NYS Public Health Law Section 25000-h we must explore further if this allergen may result in a possible anaphylaxis reaction if exposed, even if it is unlikely there will be any exposure (i.e. we don’t serve shellfish at the program) and further documentation will be required. We need to know from you and your child’s physician, if exposure to the allergen may require the administration of an emergency medication to prevent/reduce the anaphylaxis reaction. You will need to meet with your child’s doctor and discuss the reaction and evaluate if emergency medication is needed. One of the following 3 options MUST be obtained once you meet and discuss with your child’s doctor.
- If no medication is needed at BASCOL the doctor may write a statement on their letterhead stating your child’s name and that no emergency medication is needed for your child’s sensitivity/intolerance (and list what the sensitivity is) at BASCOL program OR
- Doctor can complete the OCFS 6029 and list as a sensitivity or intolerance on the form and that no emergency medication is needed at the BASCOL program.
- If medication is needed at BASCOL the OCFS 6029 and additional MAT – Medication Consent Form Paperwork will need to be completed.
